This is a confidential onboarding form to ensure that I have the correct contact information for you, an emergency contact and for you to share any other information that you would like to ahead of our first session. I keep forms for three months after our last session (in case you book further sessions) and then all information is deleted. Anything that you share in this form will be held completely confidentially.
